Why I created Second Prime

Midlife is often portrayed as a plateau, a time to accept slower thinking, stubborn weight, or quieter ambitions. But I’ve never believed that. And neither does the science.

For many adults in their forties and fifties, midlife isn’t a crisis it’s a moment of realisation: that the tools that once worked effortlessly now need updating. Not because something’s gone wrong. But because you’ve changed and you’re ready to meet that change with intention.

From NHS to GHS: What I wanted to build

When I moved from the NHS into founding GHS Clinics, it was because I saw, both clinically and personally, how much the system is not designed to serve people in the most strategic phase of their lives.

In my professional world, I watched people wait too long. Health interventions were delayed until symptoms reached diagnostic thresholds. Conversations were framed around risk, not potential. Even in private settings, care was too often fragmented, reactive, or narrowly diagnostic.

But in my personal life, something else was happening entirely.

I was surrounded by people in their forties and fifties who were stepping into their prime, not out of it. Friends were training for Ironman competitions. Swimming across oceans. Starting PhDs. Launching businesses. Writing books. Cold-water swimming at dawn. Raising teenagers by day, submitting doctoral theses by night. These weren’t anomalies. They were the vanguard of a generation refusing to fade quietly and instead, choosing to engage more deeply, more intelligently, more actively with their own health and capacity.

And when I turned to the science, it backed up what I was seeing.

The clinical data is increasingly clear: the midlife decades are not a slow slide into decline they are a strategic window in which the brain remains plastic, biological ageing can be slowed, cognitive function can be enhanced, and long-term morbidity delayed. Research on the Compression of Morbidity hypothesis (Fries, 1980; UKRI, 2024) shows that with targeted midlife interventions across exercise, sleep, stress, diet, and hormone optimisation, we can dramatically shorten the years of illness and dependency at the end of life.

This isn’t theoretical. It’s actionable. And it’s not just about adding years it’s about reclaiming them with vitality, clarity and direction.

And yet, most healthcare services still treat the forties and fifties as the unregulated decades too early for clinical intervention, too late for foundational health design. In my opinion, that is a massive blind spot. One that wastes potential. And one I was determined to correct.

That’s why I created Second Prime. It’s not just a clinical programme. It’s a system grounded in science, led by professionals, and shaped by strategy. It reflects what I believe midlife can be: not a soft decline into maintenance, but a decisive investment in the next 30 to 40 years of strength, cognition, health and identity. And, I believe, if approached with clarity and the right support, your second prime can be even better than your first.

Second Prime is a Power-Shift

Second Prime is a system designed to return authority to where it belongs – with you.

Too often, healthcare unconsciously reverts to hierarchy. I’ve watched incredibly accomplished people, leaders, academics, entrepreneurs, step into consulting rooms and feel their agency dissolve. They defer. They accept. They shrink into the role of ‘patient’, rather than remain the strategist of their own body and mind.

That needs to change.

Second Prime is built around the principle that you are the expert in your own lived experience. Our role is to help you map it, understand it, and design around it with clarity. The Brain-Body Inventory is one way we do that. It’s a proprietary tool that asks not “What’s wrong with you?” but “How is your system operating, and how can it be strengthened?”

The system works not because we take charge, but because we help you take ownership. And the research supports this. A growing body of evidence shows that when individuals have a clear understanding of their health state and participate meaningfully in decision-making, outcomes improve significantly from metabolic measures to mental health, medication adherence to functional longevity (Charles et al., 1999; Coulter & Collins, 2011; NICE NG56, 2016).

Second Prime honours that science. And more than that, it gives it structure.

The Book is a Manual for Midlife Clarity

That same philosophy is at the heart of my book, Second Prime. I wrote it to give people a framework for seeing themselves clearly, acting strategically, and staying in control, not just for a few months, but for the next few decades.

The book mirrors the clinical model we use at GHS Clinics. It gives you a structure for thinking about your health strategically. The book is organised around the same four-stage system we use with clients: Audit → Recalibrate → Build → Sustain.

  • In Audit, you’ll complete a guided self-assessment, using a version of our Brain-Body Inventory to map cognitive load, metabolic state, emotional rhythm, and identity alignment.
  • In Recalibrate, you’ll learn how to shift behaviours intelligently using small, high-leverage adjustments that unlock momentum.
  • In Build, we help you assemble a personalised Health Operating System integrating wearable data, routines, nutrition frameworks, and performance metrics.
  • In Sustain, you’ll explore how to stay adaptable, aligned and focused, adjusting your strategy over time, without burning out or drifting off course.

Each chapter includes self-reflection prompts, clinical research summaries, practical tools, and real-world examples. It’s a resource you can return to, not a book you read once and shelve.

What Second Prime Looks Like in Practice

Let me show you how this works in real life, with Rachel a 47 year old.

On the surface, Rachel was managing well: a senior academic, mother of two, physically active, and by her own admission, “not ill.” But carries a quiet concern that’s common among high-functioning adults: a creeping sense that her mental sharpness was starting to slip.

She describes word-finding difficulties, a reduced ability to concentrate during long meetings, and a loss of confidence in her short-term memory. She wasn’t in cognitive crisis but is worried.

We begin with a therapist-led intake session, which allows Rachel to articulate her concerns in context, not as isolated symptoms, but as part of a broader shift in how she was experiencing herself. She completes the Brain-Body Inventory, which surfaces additional patterns: unstable energy, increased reactivity, and disrupted sleep. This is overlayed with Rachel’s wearable data that has captured years of information around Rachel’s sleep, movement, stress, Heart Rate Variability, VO2, menstrual cycles, perimenopause symptoms, resting heart rate etc.

From there, we build Rachel’s personal multidisciplinary team: a therapist, a clinical lead, a nutritionist, a personal trainer and aesthetics nurse. Rachel asked for the PT and aesthetics lead to be added to the team. Rachel and her team meet, they review the data, contextualise it with her goals, and invited her to lead the conversation. Her objectives shaped the strategy.

One key decision was to run a DUTCH test, an advanced hormone and adrenal panel. Rachel’s results revealed elevated cortisol in the early evening (likely contributing to her 3 a.m. waking) and poor overnight melatonin production. Combined with an oestrogen-progesterone imbalance, this pointed to a clear hormonal-cognitive interaction.

We don’t stop at audit. We then move to recalibrate.

Rachel’s Health Strategy Card includes:
